Excision of the shaft: of the tibia for sarcoma; examination of a patient forty-three years after replacement by a fibular graft.

چکیده
OIl August 4, 1906, I resected tile left tibial diaphysis of a girl aged fourteen years for sarcoma diagnosed clinically and radiologically (Figs. I and 2). Tile segment excised was replaced by a graft of equal length from tile fibula, the soft tissue attacilments of which were disturbed as little as possible durillg mobilisation in order to preserve tile blood supply. My reason for undertaking conservative resection was tilat tile sarcoma was a central one, limited to tile bone and showing no soft tissue invasion. Histological examination of tile tumour showed a uniform structure. It consisted of connective-tissue spindle cells set in several directions, ‘ith an abundant vascular stroma. In se -’eral places, but mostly w’ilere the stroma was abundant and the fusiform cells scanty, there were groups of six or more giant cells. These were similar to the m ’eloplaxes of tile bone marrow, from which tlley were probably derived. There were also some areas of degeneration, where the stroma was granular and its reaction-distinct from the acidopilil reaction of the rest of the tumour-was basophil. Since tile first publisiled report of this case in 1907 it has been referred to by several writers, some of whom have doubted the accuracy of the diagnosis. Cavaillon and Alamartine

A case of replacement of the upper end of the humerus by a fibular graft reviewed after twenty-nine years.
In recent years, after resection of the ends of long bones, increasing use has been made of acrylic resins and stainless steel for replacement, to the exclusion of the bone-grafting procedures described by earlier workers. Rovsing (1910), after resecting the proximal two-thirds of the humerus for round-cell sarcoma in a man aged twenty-six, used a graft of the proximal end of the fibula to repl...
متن کاملDistal Recurrence of Periosteal Osteosarcoma After Complete Excision of Proximal Primary Tumour With Good Excision Margins
We present this case of an unusual recurrence of a periosteal osteosarcoma in the distal right tibia 2 years after a successful proximal right tibia primary periosteal osteosarcoma excision with a successful fibular graft. This recurrence lead to a right below-knee amputation.

Method. We performed a two-stage vascularized ipsilateral fibular graft transfer for segmental tibial defect following excision of malignant bone tumours.Results. We report 10 patients who had this procedure with an average follow-up of 116 months. The graft was transposed medialy on its vascular pedicle by two-stage surgery. Full weight bearing was achieved in six patients at 8-43 months postop...

Although various techniques have been described for treatment of severe nose deformities, these problems have high revision rates. Conventional nasal septal surgery may not be adequate for all cases. متن کاملOutcome of Distal Both Bone Leg Fractures Fixed by Intramedulary Nail for Fibula & MIPPO in Tibia
Background: Fractures of the distal third of the tibia are mostly associated with a fibular fracture that often requires fixation. The preferred treatment of distal tibial fracture is the minimally invasive percutaneous plate osteosynthesis (MIPPO) procedure. However, there are no clear cut guidelines on fixation of the fibular fracture and currently most orthopedic surgeons use a plate osteo...
